How
Do I Register With The Practice?
Please bring in your medical card
and the receptionist can help you to complete it. If you do not possess a
medical card, please ask at reception for an application form. Temporary
residents are registered on a special form at reception.
Will be given a health and
medical history questionnaire to fill in and an appointment for a health check
by the Nurse. This is important as it allows us to get to know you better and
collect important information on past-medical health, current problems and
medication. It is also an excellent time to ensure that all immunisations,
contraceptive requirements, cervical smears and blood pressure measurements are
up to date. There can sometimes be a delay in obtaining your records from your
previous Doctor and therefore the new patient information is vital to your new
Doctor.
Visitors to this country will
normally be charged a consultation and prescription fee. Those from certain
countries may be entitled to NHS treatment as Temporary Residents. Please ask
at reception for details.

The
Change
of Address or Telephone Number
If you
change address or telephone number, please inform the receptionists as soon as
possible. This helps keep our records and computer up to date and ensures
appointments are sent to the right address. Remember to inform the hospital
appointment desk as well.
Once authorised by your Doctor,
requests for repeat prescriptions may be made, by telephone, by post or
personally at the Surgery. At
The two local Canford Heath
Chemists have introduced a prescription collection service, where you drop in
your computer slip to the Chemist and then pick up your tablets from them three
days later.
With our new automatic telephone
call distribution service, it is now possible to leave requests for repeat
prescriptions by telephone. Please be clear in your requests, referring to the
drugs you require by name and not by colour, size, shape taste or smell!!
You may ring the surgery for the
results of tests after 11am but please remember the following points:
1. Results can only be given to the specific patient to whom they apply. In the interest of confidentiality please be prepared to identify yourself. Please do not phone for results for friends or relatives unless we hold written authority to discuss their case with you. We hope you will be understanding in this matter but we do not have a great deal of discretion in this area.
2. You will need to know the specific test results that you require. Some tests take longer than others to process. If you have had several tests then the receptionist will not necessarily know whether all the results are back unless you can identify them.
3. Please do not expect the receptionist to have any medical knowledge. They are instructed only to tell you if your test is indicated as normal. If any test is abnormal, requires further discussion or needs further interpretation then we can refer the matter to your Doctor.
Any patient may request a health
check by our practice Nurse. Please telephone for an appointment. New patients
signing on with the practice for the first time are welcome to request a ‘Registration
Check’. This would be highly recommended for patients with a complex
medical history.
Do please make use of the height,
weight and Blood Pressure measuring machine in the Mitchell Road waiting room.
This will produce a paper print out of your vital statistics which should be
handed to the reception desk so the information can be entered on your
computerised clinical record.
If you care for a relative or
friend and would like to participate in our carers group, please inform reception
and we will involve you in our carers group which can be very helpful in the
provision of support for what can be a very difficult and demanding role.
If someone dies, call a Doctor.
There is no need to move the patient. It is a legal requirement for a Doctor to
confirm that someone has died. If the Doctor has recently seen the patient a
death certificate can normally be issued.

operate a formal complaints procedure. If you have a complaint, please contact
our Practice Manager, Mrs Hazel Clarke, who will investigate the matter and
inform you of the outcome. If your complaint is on behalf of another person, it
will be necessary to ask you for their signature authorising us to discuss the
matter with you.
