New Patient Registration

How Do I Register With The Practice?

Please bring in your medical card and the receptionist can help you to complete it. If you do not possess a medical card, please ask at reception for an application form or click on the Registering Online tab (above) where a form is available either complete online or print. Temporary residents are registered on a special form at reception.

New Patients

Will be given a health and medical history questionnaire to fill in and an appointment for a health check by the Nurse if requested. This is important as it allows us to get to know you better and collect important information on past-medical health, current problems and medication. It is also an excellent time to ensure that all immunisations, contraceptive requirements, cervical smears and blood pressure measurements are up to date. There can sometimes be a delay in obtaining your records from your previous Doctor and therefore the new patient information is vital to your new Doctor.

Foreign Visitors

Visitors to this country will normally be charged a consultation and prescription fee. Those from certain countries may be entitled to NHS treatment as Temporary Residents. Please ask at reception for details.
